First Time Minor Passport!

Requirements:

  1. TWO (2) FORMS FOR FINGERPRINTS (Download link at the bottom of the page).
    • The Fingerprint 1 form must be NOTARIZED.
    • The Fingerprint Form 2, DO NOT NOTARIZE.
    • Make sure the information on both forms is written in blue pen, in legible print.
    • It is necessary to use blue or black ink to stamp the fingerprints. IF YOU ARE UNDER 10 YEARS OLD, THE FINGERPRINTS MUST BE FROM THE PARENT RESPONSIBLE FOR PROCESSING THE APPLICATION.
    • In the signature box on the forms, you must write the minor's first name and last name. (Do not sign)
  2. One (1) Copy of the Birth Certificate from the year 2022 onwards with QR code. If you do not have a valid birth certificate, you can process it with us for an additional cost of $20.00 dollars.
  3. One (1) copy of the Identity Card and Voter Registration Card of both sides of the parent responsible for processing the application. The name on the passport will appear as it appears on the Birth Certificate. For any name change, you must first make the change on the Birth Certificate.
  4. Affidavit indicating that this is the first time a Dominican passport has been issued in the minor's name, signed by the parent responsible for processing the application. (Download link at the bottom of the page)
  5. Authorization for a minor requesting passport issuance, signed by the parent responsible for processing the application. (Download link at the bottom of the page)
  6. A passport-sized photo (2×2).
    -NOT PHYSICAL, send it digitally to info@consulatedrwest.gob.do
    (Take a photo with a cell phone or camera and send it to the email after submitting your physical documents.)
    -Photo requirements include: a white background; both ears and the forehead must be visible; hair must not cover any part of the face; no smile; no glasses, no earrings, no chains, no necklaces, and/or accessories. Clothing must have sleeves; shoulders must be uncovered.
  7. We only accept money orders and/or certified checks made payable to the Dominican Consulate.
  8. Include a prepaid (pre-stamped) and self-addressed envelope for returning documents. We recommend using UPS or USPS.

  • Cost: Certified check or money order for an amount of $263.00 dollars for 6 years, $338.00 dollars for 10 years (14 years and older) in the name of the Dominican Consulate in CA
