Minor Permit Requirements

Requirements:

• Permission letter (see the sample PDF on the bottom of the page). The signature of the parent and/or guardian must be notarized by a Notary Public
• 2 photographs of the child (passport type)
• Copy of the Child's Birth Certificate
• Copy of the Child's Passport.
• Copy of Flight Itinerary
• Copy of passport and valid ID (e.g. driver's license) of the authorizing person.
• Indicate if you are traveling with a flight attendant; otherwise, please send a copy of the passport and identification of the person traveling with the minor.
• Include a prepaid (properly stamped) and pre-addressed envelope for the permit's return. We recommend using UPS or DHL (we are not responsible for shipping costs).
• Please send the documents indicated above, together with a certified check or money order payable to the Dominican Consulate in California, to our address: 500 N. Brand Blvd. suite 960 Glendale, CA 91203.
