BIRTH CERTIFICATE TRANSCRIPTION REQUIREMENTS

1 Birth certificate duly apostilled by the Secretary of State of the state that issued it. (The apostilled certificate must be submitted in physical form; it must be the original, not a copy. Please do not remove the original staples.)

2 Copy of passport (if minor) or foreign driver's license (if adult).

3 Dominican father or mother identification:
Copy of your current Dominican Identity Card and Electoral Card, both sides. (MANDATORY)
Note: Birth certificates of either parent CANNOT be used.
It only takes a parent to be a Dominican citizen.
Process duration: 2 and a half months.

Send two Money Orders
• Money order 1: Made out to the Dominican Consulate for $145.00 (DOMINICAN CONSULATE). Translation of the deed.
• Money order 2: Made out to the Dominican Consulate for $55.00 (DOMINICAN CONSULATE). Transcript of the minutes.
