The signature of the parent and\/or guardian must be notarized by a Notary Public<\/li>\n<li>2 photographs of the child (passport type)<\/li>\n<li>Copy of the Child's Birth Certificate<\/li>\n<li>Copy of the Child's Passport.<\/li>\n<li>Copy of Flight Itinerary<\/li>\n<li>Copy of passport and valid ID (e.g. driver&#039;s license) of the authorizing person.<\/li>\n<li>Indicate if you are traveling with a flight attendant; otherwise, please send a copy of the passport and identification of the person traveling with the minor.<\/li>\n<li>Include a prepaid (properly stamped) and pre-addressed envelope for the permit&#039;s return. We recommend using UPS or DHL (we are not responsible for shipping costs).<\/li>\n<li>Please send the documents indicated above, together with a certified check or money order payable to the Dominican Consulate in California, to our address: 500 N.
