(It is essential to present it.)<\/li>\n<li>One (1) Copy of the Birth Certificate from the year 2022 onwards with QR code. If you do not have a valid birth certificate, you can process it with us for an additional cost of $25.00 dollars.<\/li>\n<li>One (1) copy of both sides of your Identity and Electoral Card. The name on your passport will appear exactly as it appears on your Identity and Electoral Card. For any name change, you must first make the change on your ID card.<\/li>\n<li>A passport damage affidavit stating the circumstances surrounding the events at the time the passport was damaged. (Download link at the bottom of the page)<\/li>\n<li>Two (2) 2\u00d72 passport-size photographs.\n<ul>\n<li>The photo requirements are: a white background; both ears and the forehead must be visible; hair must not cover any part of the face; no smile; no glasses, no earrings, no chains, no necklaces, and\/or accessories. Clothing must have sleeves; shoulders must be uncovered.<\/li>\n<\/ul>\n<\/li>\n<li>We only accept money orders and\/or certified checks made payable to the Dominican Consulate.<\/li>\n<li>Include a prepaid (pre-stamped) and self-addressed envelope for returning documents. We recommend using UPS or USPS.<\/li>\n<li>Please send the documents indicated above to our address: 500 N.
